PAYMENT TERMS

ALL LAMPS MUST BE PAID FOR IN ADVANCE OR GUARANTEED BY A CREDIT CARD OR SCHOOL, HOSPITAL OR CORPORATE, PURCHASE ORDER. CHECKS OR CREDIT CARD PAYMENTS WITH PREPAID ORDERS WILL BE PROCESSED ON RECEIPT OF THE ORDER.

WE WILL ACCEPT PERSONAL CHECKS, FROM INDIVIDUAL STUDENTS. FOR ALL ORDERS GUARANTEED BY CREDIT CARD, THE CHARGE WILL BE PROCESSED WHEN THE ORDERS ARE READY TO BE SHIPPED UNLESS ANOTHER FORM OF PAYMENT HAS BEEN RECEIVED PRIOR TO THE SHIP DATE. WE DO NOT PAY SHIPPING ON ORDERS PLACED ON THESE TERMS.  

TERMS FOR SCHOOL, CORPORATE, OR HOSPITAL PURCHASE ORDERS ARE PROFORMA (PREPAY) ON THE FIRST ORDER AND DUE ON RECEIPT OF THE MERCHANDISE (MAXIMUM OF NET/10 DAYS) ON REORDERS (unless otherwise negotiated prior to the order placement). A LATE FEE EQUAL TO 2 PERCENT PER MONTH WILL BE CHARGED ON UNPAID INVOICES, BEGINNING 10 DAYS AFTER PROOF OF DELIVERY IS RECEIVED FROM THE CARRIER. ALL LAMPS WITH KARAT GOLD EMBLEMS MUST BE PAID FOR IN ADVANCE WITH NO EXCEPTIONS. LATE PAYMENTS MAY RESULT IN YOUR SCHOOL, CORPORATION OR ORGANIZATION BEING PLACED ON PREPAY STATUS FOR ALL FUTURE ORDERS AT OUR DISCRETION.

WE WILL SUPPLY A W-9 TO YOUR ACCOUNTS PAYABLE DEPARTMENT, AT YOUR REQUEST.  VENDOR APPLICATIONS MUST BE RECEIVED PRIOR TO THE ORDER BEING PUT INTO WORK BECAUSE IT ADDS TO THE WORK LOAD AND MAY RESULT IN DELAYS FOR YOUR ORDER.

SHIPPING

Orders for less than 12 plain lamps (no single decorated lamps except extra large lamps please)are accepted when ordered online, and will be billed for the  shipping & handling charges.                       GIFTS OF HEALTH will pay shipping via UPS & USPS ground to the school, hospital or other commercial address, for  group lamp orders of 12 or more, when payment is received at the time of the order. (Credit cards will be charged when the order is placed.)   UPS CHARGES AN ADDITIONAL $10.00 HANDLING CHARGE FOR GROUP ORDERS IF A RESIDENTIAL SHIPPING ADDRESS IS REQUESTED. This charge will be added to any order/invoice that requires lamps be sent to someone's   home address.             SHIPPING & HANDLING CHARGES of 1.25 per Medium or Mini Lamp will be added to invoices with purchase orders requiring terms.
We do not pay shipping on PLAIN LAMP RUSH ORDERS that request a delivery date in less than 14 business days from the date of the order. Nor, do we pay shipping on rush orders for decorated lamps that do not fall within the standard shipping times. Personalized Lamps with emblems must be ordered 10 weeks in advance. Lamps with new emblems or personalization only, must be ordered 8 weeks in advance and reorders must be ordered 6 weeks in advance. Anything less is considered a rush order. 

All orders for plain lamps that require shipping in less than 5 business days will incurr a $25.00 Rush handling charge.   Rush charges for decorated lamps will be at our discretion and   based on our ability to meet the rush.

BROKEN LAMPS will be replaced. Please call for information if you receive a broken lamp. Please allow up to 90 days for replacements of decorated broken lamps.
